STEP ONE - You want some more information

Call our Intake Line at 847-386-6560 x307, email us at info@kickstartptn.com or fill out this form.  One of our Clinical Directors will call to discuss your child and the services that we can provide. If ready to move forward, a time will be scheduled for an initial visit, you will be emailed an Intake Packet and we will ask you for your insurance information so that we can check your coverage. You can also download an Intake Packet here.

STEP TWO - You are ready to meet us!

Once you set up a time, come on in to meet a therapist at one of our clinics. You can either come alone or bring your child. (Just note that if you do come solo, we will likely set up another time to meet your child, as well.) Be sure to bring your completed intake packet along with you. Your visit is a time for you to receive a tour of the facility, ask questions and determine how we can best individualize programming to the needs of your child. 

STEP THREE - Scheduling therapy/group

Our therapists will work with you to create a consistent schedule for your child's therapies and/or programming.
